Agreement to Mediate
Please read this as it goes over how mediation works. The mediator will answer any questions you may have. This form will need to be signed by you. The mediator may sign it remotely with your permission, if needed.
Intake Screening
Everyone who attends an orientation/mediation session must complete this form. It gives the mediator an overview of your case. All information is completely confidential and private. The only thing the mediator is required to report is any allegation of child abuse and/or neglect. Please complete this form and send it back to us.
Required Support Documentation
The information needed to calculate child and/or spousal support. Please be sure to provide this information to the mediator prior to the mediation appointment.
